The Weekly Schedule Contract is due each weekly on Mondays by 5:30 PM. Parents can print off the form, download and save it, complete the form at the center or email in a schedule. Send completed contracts to

Complete the form and after it is turned into the office, we will use our resources to determine if your family is eligible for free or reduced meals.

This form lists who may pick up a child from the center, as well as the pick up person’s contact information, as well as relationship to the child. Parents, grandparents or any one who will regularly pick up a child from the center should be listed. A child will only be released to people on the release form. Additionally, there is also a place for other relevant information and anyone who is not allowed to pick up specifically. The only way a biological parent will be denied access to their child, is with a court order. The law requires biological parents access to their child, unless Rainbow Land has a copy of the court order on file.

The permission form lists several activities and items that need parent consent before children can participate. Parents need to mark yes or no for each item listed.

DHS requires Rainbow Land to have emergency medical and dental information on file at the center. This form lists all the required information that we may need incase of an emergency.

All children must have a completed physical to attend the center.

The center needs permission to use sunscreen on children. Parents can bring personal sunscreen or opt to use the center’s sunscreen.

Emergency Information for the room’s first aid kit that goes everywhere the children go, in case of emergency or need for first aid.
